For any disabled adult*, transition services are vital. No matter the age of the individual, transition planning is essential and it is for you now wherever you may be. Preparing for adulthood is complex and nuanced.
It can be daunting to know even where to start.

At Able Transition, we provide a path forward for individuals with disabilities and autism*. We build unique and comprehensive life-long person-centered plans for every area of adult life. We build the plan, create targeted goals, give recommendations, and ensure you have every resource to ensure high-quality and successful adulthood. We are bridge builders to all that you need!

*We use identity-first and person-first language dependent upon each clients preference. Language is important, and ensuring each client feels respected by the language they prefer is essential to our values.

Long-term care planning is the most essential component of what we do. We collaborate with clients to build out their dreams and goals. Working with clients we take a holistic approach.
ABOUT US

Able Transition began out of a need our founder, Kenzie Clark, saw as she engaged with families and students in her role as a Special Educator. With a Masters in Transition, she has the education, experience, and passion in transition. Through this work she has seen, first-hand, the importance and need for planning, preparing, and collaborating with individuals and their families for a high-quality future.

Transition is so much more than preparing for education, employment, and independent living. It is a complex process and we are here to provide a unique roadmap forward for all of our clients.
